The quick outfit change wheel is a newer feature and thus buggy. It also yeets glasses every time. If it isn't sticking, it's probably a bug. Feel free to send in a bug report ticket about it so they're aware of this issue.

I've heard that toolskins themselves are bugging out people's tools as well and making them unusable after this last patch

Nope, you can only place an item on one plot. If you want to place something on multiple plots you need to get a duplicate of that item
