You probably did not actually play ironman-mode (because there's currently a bug preventing it from applying correctly, there's a workaround to get it working though).

Ironman-mode is bugged.
If you create an ironman save, it's actually not an ironman save but just a regular one.

Workaround;
(Only works with a controller)
Create a new save (enable ironman-mode before), then you're at the difficulty select screen, hover over your desired difficulty, then you press X (on an xbox controller) to enable ironman-mode (works with any controller).
Basically you have to enable it twice, once on the first screen & once on the difficulty select screen.

If you don't wanna replay through it again, find a savefile (haven't tested it, but should work to get the achievement).

For a physical model you're only going to find that as either a fixed attachment on weapon replacers or as an option for addon weapons with extra attachments. For functionality Switch Illuminator Attachments still works.

Well I'd recommend you first join the modding discord: https://discord.gg/TjyfbXTU28 go to the how-to-create-mods channel and find the unofficial modding guide and in there will be .bat files for paking, you'll need unreal engine 5.3.2 specifically for the unrealpak files to be able to use the .bat files.

After that for paking files:

VO folders/files are now located in pakchunk0-Windows.pak you can open this if need be if you need the actual files for any reason by downloading and using Fmodel also available in the unoffical modding guide. As long as your files are the correct name and format (.ogg) you don't actually need the originals if you have those already for any reason I could think of.

Creating your mod folder means you'll have to follow how the path is in the game files so for example if you want to make a mod folder to pak for your voice mod and it's only replacing Judge's voice then your mod folder would look something like this: pakchunk99-Mods_ExampleVOmod_P\Content\VO_PC\SWATJudge

Than you'd simply have your files placed in that SWATJudge folder and then drag the entire mod folder over the .bat file to pak and put it in your paks folder.

Everything seems right so if you really don't have 9 Banger under grenade in your munition slots I can only assume for whatever reason one or more of those files isn't being read by the game.
You can test this by: while running the game attempt to delete or modify/rename the files as windows will block the action if they're considered currently being used in the game.

Right lol. For me it's in deployables section, bottom of the list in grenades. 9 bangers are great but not as effective as 1 or 2 stingers in the same room. We use them in our playthroughs but more as a distraction to get in and make some commands not to subdue prior to entry.

Load order smaller number loads before larger number, larger number overwrites smaller number if they both modify the same files generally for most mods you won't need to change this.

Do note that the load order is in alphabetical order so depending on the number it doesn't always actually load after or before just because it's larger or smaller it's best to just add or reduce the amount 9's that or just change the numbers to letters.

Within that last folder is where you should place all your VO folders. These should match exactly with the ones in the game files, if anything is misnamed or not set up properly, it won't work. Additionally, depending on how old the mod is, the game's voice files may have changed since the mod was created, and the files in the mod may no longer have the correct names or even be the correct file format.

Yea those files are in the incorrect format older versions of RoN used .wav files but current ron uses .ogg files for it's audio format and with that this also implies that even if you convert the files like I said earlier some of those file names may no longer be correct.

The only gunfighter file you had installed was the Anim file which only covers the animations if you wanted more than you'd either have download the rest of the individual files or the AIO file.

Make sure you have 46 windows.pak/.sig files in there. You should have both .pak and .sig going from 0-22 though if your crashing was caused by one of those missing usually you wouldn't be able to launch the game at all.

Idk maybe just change the name of your driver there because that really isn’t any longer than what it would be if you had it installed on the C drive this is in fact probably shorter since it doesn’t go through programs files (x86) as well.

You could also just rename the pak file the only important things that need to remain is the load order number and the _P at the end but this wouldn’t be a very convenient solution should this keep happening.

Yea, the references to CSAM in the uncensored content from the game need to be removed from the mod, as Nexus does not want to host such content due to the potential risk of being fined. To quote the Nexus CM (the same person named in your screenshot for hiding the file):

Now, whether the content is actually illegal under these laws is a bit of a gray area, I’d say. From a company or business perspective, it's understandable why Nexus would want to remove it to avoid any potential liability or fines for hosting it.
