I can confirm it actually can be locked behind the skull doors - I would have had a 12 minute run if it wasn't for all the running around I had to do to find the Satyr Sack. The good news is ...I finally faced King Vermin for the first time, so got a codex entry.

To be clear on that, the sack can appear in any tunnel after the first one you pick. So there's a 0% chance it's in the first tunnel, then it's about a 54% chance that it's in the second tunnel. And then it goes on like that.

Miniboss tunnel or not has no effect on the chance.

So you can usually assume that it's in the second or third tunnel that you take. That works out more often than not.
