Nominative is the case that you use for the subject of a sentence, and it's also the "dictionary form", as in, the default form when you write the noun by itself.
So like for example, for the word "Fisch", you learn it as "der Fisch" because it's masculine.
The "der" is the nominative article for masculine nouns.
And you learn the nominative articles by just memorising them with the noun when you learn a new noun.

Sure. The basic idea is that for most verbs, the direct object is accusative. There are some exceptions though. A small number of verbs instead go with nominative, dative, or genitive. But you just have to memorise those exceptions.

Its like if you were tested on identifying animals by their picture and got 20/20 correct then believed yourself to be "an animal expert" that can identify any animal on earth forgetting that there are thousands more you were not tested on. You are tested on what you know for a very small section of overall language
Honestly there’s not really a tested manner in which someone can prove they’re proficient in a certain language
The leveled tests people take help one have an idea of what concepts to learn and what topic of vocabulary one should have in mind (like CEFR) and a leveled test like C2 would be a decent indicator of the level somebody’s in, but it’s not completely perfect
