Could be a glitch due to rendering. Occasionally, the game engine fails to reload the "damaged" state of an object and defaults it to its "repaired" state upon initialization. The mission logic sees the object is now functional and checks the box.

If you're next to a extreme cold/hot/rad/toxic planet, your hazard protection depletes at rate like as if you're in extreme cold planet

Well, yesterday i felt bored in this game. Like, REALLY bored. Because i thought it was a good idea to go for catching all legendary fish with the rod. After 3 1/2 hours i managed to get something but was not even close to achieving this. I just clicked leftclicked for hours. thats it. That is boredom in its purest form. It was wonderful.

If you just warp into the system you normally just discover the system itself but planets remain unknown.
The freighter scan is similar to scanning them from space, except it adds them to the discoveries list, and shows the resources and number of fauna (if any). Still need to land and scan to record actual species, but you can look back at the system later and at least the planets will be shown.

The mod just adds the planetary probe scan thing to the mission radar you can build in the corvette.

Otherwise I had to actually go to each planet and moon in the system.
