Not only do you have to stack it up 4 times asap, which means trinket is pretty much unusable on any fight that lasts less than 3 minutes - you also get the bleed debuff and movement speed debuff which are terrible

and on top of that

you have to get the full 30s value of the final rune and get the explosion on the boss and the boss has to die soon after that happens

or you're losing a lot of value

if your final rune explodes and the boss is alive for an extra minute after that, you've just wasted a minute of 4 stacks of Scars

So it's very specific when you want to use the final rune to get the maximum value out of the trinket and it's not super reliable or easy to predict.
