Meh, the game design (since original, not a GAMMA/modpack thing) is full of bullshit things. I've died to enemies spawning next to me when I move to another place (both with and without fast travel) or to the inmortal enemy bug, etc, etc. Xray is famous for these kind of thing so people shouldn't take this so seriously, specially considering it's a singleplayer game so you can edit gameplay videos all you want and hide things to pretend you are a PRO player but who cares, is a game accessible for anyone not Dark Souls.
