It’s possible to fix and is a pain, easy “safe guard” is just to save || before you enter the facilities || but as far as I know, it’s caused by || entering an alien portal while leaving your prawn suit in the alien facility. Unloading the facility and your prawn gets stuck in it, so either go in portals WITH your prawn, or just leave your prawn outside the facility ||
