i think i know what's wrong but fixing it involves using special tools from oculus
likely that something was corrupted, so you would need to reflash the board. doing so would both void your warranty and probably lead to your meta account being banned after you log in because the firmware isn't signed by oculus (a sign of a modified/hacked console)
