Hellmode forces 2 heat modifiers which are Jury Summons (extra enemies) and Calisthenics program (enemies have more hp), so you dont speedrun with those unless you're running certain categories like 50 heat

Basically does he have to do something for someone to die?) or does he only show up when people die and escort them to the underworld?

I disagree. When youāre killed you die, but depending on how youāre killed a lot of things could happen before you die (and I donāt mean that in a religious way). In the same regard all pain hurts, but some pain hurts differently than other pain.
Personally I agree that Thanatos does not kill, but for the sake of the argument I ask again; Does Thanatos kill or just show up?

Thanatos is in charge of Death. Hades is in charge of the dead. Hades does not decide who dies and when, he merely guards those who have died. Thanatos does not decide who dies - he merely escorts them to Hades. The Fates decide who dies and when. Hades and Thanatos work in tandem. Thanatos is the elder of the two, as son of Nyx, and more conceptual than anthropomorphic. So he is probably more powerful due to his age. Thanatos seems to work for Hades, as when he was sent to fetch Sisyphus, but I don't think Hades can compel him to do his will.

This is why people can return from Hades and have a human life.
But that is a difficult subject because they are still Shades according to Homer.

I wouldn't do unmodded when I speedrun, not that I do that often. Main difference is that you'll reset a lot more in unmodded because of things that can't happen to you in modded.
And resetting takes longer.
In modded, you can just do a button combo to immediately quick-reset.

Tooth is reasonably consistent overall
I go for Acorn if I have a low hp pool/ mediocre build, as a gamble, because if Acorn works out in your favor it's massive value
