Click the hamburger-menu at the top-left of SteamVR's desktop app, go down to Developer > Developer Settings, then Remove All SteamVR USB Devices

Unplug the Index from your computer, then plug it back in and wait for Windows to recognize it again, then try SteamVR

I used to be BigKidLol, however Someone by the name of Legacy has stolen my account and changed my name to floyed_monkey.
