normally you go to a monolith and then if you pick the right dialouge you can leave an offering

and it shows you where a portal is

Finding the monolith might be the harder part, but once you find one put down a save beacon by it and make a save point that way if you interact wrong you can just reload and try again. Need a race item to have it locate the portal, and not a bad idea to leave a save point by the portal if its a system you commonly use or else a simple base nearby to get to it later.
