Yep: I'm in a two-planet system that has ONE portal on the second planet and no portal on the first, even though the game says it's there. If I try to portal to what should be the address for the first planet, it dumps me right back on the second planet at its portal.

The game says the portal is on the ocean floor, as it has been for the other ocean planets I've discovered, but when I visit the markered spot there's just a prepped site and no portal. Damnedest thing.

And when I arrived there's no "monolith" marker for it. It's really not there.

As evidenced by the fact that I can't just assume that it's there and portal to its glyph address.

Also, if you insist on finding the envoys, make sure you find explorers, and pay attention to the region of the star, every region has one single guild in them, so if you find a star with not-explorer, take a note of its region on galaxy map and don't visit any stars of that region

If you do find explorer, do the same except visit every star in the same region, repeat with new region after you run out of stars

You can get them to all having matching ships, but it's a little tricky. The game wont let you hire the same pilot twice, but it will allow the same ship multiple times.

What you do is hire the first npc that lands with the ship you want, then wait for a second copy of the ship to show up. When it does, it'll be the same pilot so what you have to do is buy the ship for yourself, then switch to that ship, and trade it with a different npc pilot and hire them to the squadron before they fly away. Repeat until you have a full squadron of matching ships.

Like things keep disappearing in my base, or the colors are off

The refiners are really glitchy to the point where I almost never save and quit the game if there are still stuff in them, I wait until they finish, take them out, then savee and quit

First I thought the stuff was nicked by a visitor on the anomaly lol

(because it happened on a corvette with refiners and I was doing moulds)
