Yep, youd need to make world react and adjust in many ways: Ie, mob populations, factional warfare, consequences for death. Which would be a very different kind of game.

Not too far. Just work on leveling, finishing the campaign through Revendreth, choosing your covenant and helping them out, then gear via normal dungeons until you can start gearing via heroic dungeons
