I'm like 90% sure it has something to do with shields, recently someone found a bug that basically causes bullets to do full damage through your armor when they pass through your shield, meaning a collector kit could get shredded by an FMJ AK-74 if they were using a shield for example

maybe the scav shooting you through your shield bugged the game in the same kind of way to where they can do headshot damage to you, especially considering the scav was using a rifle
