My understanding is that when a CEO (in US) receives stock options as part of compensation, they do not pay taxes on value of the stock options themselves when received. They pay taxes when they sell the stock above the option strike price for capital gains tax. So summary, they are paid in stock equity, and pay tax when they sell the stock for gains (while avoiding a lot of taxes of normal salary income).
