Its randomly occurring issue that has nothing to do with platforms. This happens to me even when I try import my own codes that are freshly created. Only solution is to try and make new codes which means making the face from scratch again.

On the bottom right of the screen while on like face presets, theres an important button, you have to save the QR code for the face you want first to your phones gallery. The. Click the import button on the game, then you’ll get a QR to scan, from there you import the photo of the face preset QR code you saved earlier

Make sure you crop the face preset QR code

Save the photo from your PS5 app. That way the QR code is clear enough
