For the Perilous Focus penance, open areas are your enemies. Places like hallways and choke points are your friend. Take Battle Meditation and Perilous Combustion (for mixed hordes) talents near the top; of course Scrier's Gaze. Things like ER (empyric resolve), by crack of bone, and tranquility through slaughter help a lot. You can do it with guns , staves (voidstrike or purg), or melee (illisi or DS). If you can get a horde alone, you're in a good place to try for it. Enemy density is also your friend; this tends to translate to harder difficulties.

There'a also warp charge purg, but you want to get max stacks on the boss, then shriek, then melee it with a a DS4 that has unyielding and uncanny strikes on. Switch back to purg to apply more soublaze so all of your stacks don't decay (purg can only maintain 15 sb stacks without crit).

Once you're experienced enough to try clutching, I would swap toughness regen speed for extra toughness and health, and maybe a third gunner res if you particularly hate them
