From what I've seen kingdoms will start a new village based on population of the last city they started. Usually around enough housing to allow for 30 population. Was the same pre beta too. However, city zones do not spread as fast due to leaders direct involvement with expanding it now.

It heavily depends on a lot of things. How many units started the civ, which world laws are enabled, how much resources they have/have access too, etc

It can also be how lucky they are in terms of subspecies genetics. From what I've seen, it tends to be mostly better than what I'd call the baseline, but it can sometimes be worse in key areas like stamina and health
