Higher Scav Karma makes your Scav have better stuff on him more often and once you reach 6+ rep Bosses are no longer hostile to you when you go near them as a Scav

Lower Scav Karma will make your Scav less lucky and at some point your rep will be so bad that all AI Scavs will shoot you on sight

Also your Scav cooldown is affected by your rep, if your rep is that low you can actually be waiting hours instead of minutes between Scav runs

my current set up i use is a docs case, a one slot ak gas tube with a hand guard that i can put a bunch of attachments onto so i can have extra space and syphon all the attachments off of people i kill, alu splint, afak, painkiller, cms, and then depending on my kit and bullets im using a hemostat or a stack of ammo, i find this to be a very good setup mid to late wipe but earlier id use worse meds to be able to toss a bunch of task and hideout items in without losing your good meds if you die
