I waited until about 26 minutes, Popped a red flare, Popped a Yellow Flare, threw green smokes, and periodically shot into the air.

No player scav came, but an AI scav walked into the corner, and sat in the zone staring at me through the corner of the wall so he couldn't shoot. Happened on Woods too, except I put a white smoke ontop of myself and it just walked in and couldn't see me to shoot me.

his looks like a GPU driver crash/TDR (both monitors go black → CTD). Check Windows Event Viewer → Windows Logs → System for “Display / nvlddmkm (NVIDIA) / amdkmdag (AMD)” (often Event ID 4101). Then try: run Tarkov in Borderless/Windowed (not exclusive fullscreen), reset NVIDIA/AMD graphics settings to default, disable all overlays (Discord/GeForce/Steam) and Afterburner/RTSS. Test with ONLY one monitor connected + HDR/VRR (G-SYNC/FreeSync) OFF. If it still happens: DDU clean reinstall GPU driver, remove any OC/undervolt and disable XMP/EXPO for testing. Also run a RAM test (MemTest86) because raid loading black-screens are often RAM instability.
