hmm ok. initially im thinking either gpu related or storage related. if you didn't already id just make sure you are using something like ddu to completely remove the old driver before installing a new one in safe mode. in the case of storage I know you said you did the file repair but id also try a chkdsk scan on the boot drive from the recovery environment terminal
