@thorn turtle The priority for the conversation is weird even if you meet the requirements. Even if you've invested the necessary titan blood the conversation can still get blocked by story stuff. Try killing yourself early on in a few runs to see if that helps trigger it.

Yes it is. The unlocking is an investment of 1 (in case of the second aspect) or 2 (in case of the third aspect) titan blood, and will count towards the requirements for hidden aspects.

In that case, "expire" probably means that you got your guaranteed boon. It works like a single-use item. If you look closely, you will see a 1 and a circular arrow beneath it before you got the boon, which disappears after.

Death Defiance has three ranks, and it is like a consumable item. If you hit zero HP, you lose one of them and get healed back to half HP (without upgrades).
You can buy them back for 200 obol apiece at the Well if you get lucky.
The item will not always appear, so one shouldn't get too reliant on it
