Okay, so how does the damage calculation for the ||maimed || status effect work, exactly? It says a 25% increase in damage taken, however when I hit an inflicted enemy with Athena's Attack, the damage goes from 103 to 118, which is... most certainly not 25%. However against that same enemy a Doom effect that would normally do 100 damage just straight up does 125, which is an increase of 25%.

@brisk trail percentage is additive. the total attack damage is athena's % added to the ||maim|| % of your base attack damage.
base doom damage in ur example is 100 so 25% is 125. im assuming ur base attack damage (without athena) was 60... hence an increase of 15.

i don't think they've announced whether Hades is "done", all they said is that to keep alert and they'll talk about next steps of SGG in early 2021
the story and gameplay setup of Hades feels like DLC (even paid DLC) could totally be in the works
new storylines, new aspects, etc.
but that's anyone's guess
SGG might also be like "we think Hades is complete and are gonna tackle a new game" and hey even though I'd be sad that there's no more, would be excited to see what they come up with next! ofc we'd be thrilled if they said DLC/content updates would continue in Hades though!

I have about 20 hours in the game and have beaten the final boss already, and have spoken with Achilles every time possible. Is there another step required or is it actually missable?

Though the Codex is obtainable at any time after Zagreus' third escape attempt, Achilles may not hand it over right away - multiple conversations may be required to trigger the correct event. The Codex cannot be opened prior to obtaining it.
I kinda talked to him after every atempt. So just remember your old fellow sparring partner each time your back👍
