wish there was an easier way to help you. this tends to be a symptom of how the dialogue system works, genuine cases of the narrative being glitched are rare. without being able to see what dialogue you've gone through already its really difficult to suggest where you may be stuck at. doing more runs does tend to eventually solve the issue but i absolutely get how thats frustrating when you feel like something isnt working right. you might try going after other characters who still have new things to say and see if swapping prioritization opens your dialogue pool any

ok so whenever a chaos menu opens, it'll make up a list of eligible boons. even if its conditions are fulfilled, chaos defiance has to pass an additional 20% chance (i think it's 20%?) to make it into that list
once the menu has its eligible list, it'll grab an item from that list (removing it), repeat twice, we now have 3 boons

so basically 20% chance to be eligible then from there it has to be selected normally. the thing that makes this really awkward is that's a hardcoded chance- it doesn't work like rarity normally does, yarn does nothing, egg does nothing, god's legacy does nothing

so chaos defiance is weirdly rare and there's nothing you can do to help w that
