Is the only way to get nightmare is by doing 12 fear for that well towards the end? Or the rival challenge?

The shade merchant offers 1 nightmare / night, for 1500 bones. You can also command x2 of them for x5 Charon cards

There are a few different ways to get nightmare. Testaments from fear runs (shown before a run as you set up vows), you can buy it at the Crossroads, get it from prophecies...and a few other ways

That's why you gotta be careful and keep track of Vow of Denial stuff, keeping in mind these following important caveats:

1. Duo and Legendary boons are NOT guaranteed to appear even if you meet the prerequisites and there is nothing else left. They will ALWAYS only be a %chance of appearing.
2. If you don't meet the prerequisites for other boons, they will also not appear for you, even if they aren't crossed-out in the codex.
