Gotta make sure that your audio settings are set to Voice and that you’ve also set whether you want Voice Activation, Toggle, Push to Talk, etc. some people forget to set both

But just in case this is probably the reason, you have to hold the talk button the whole time while speaking through Spirit Box, Ouija Board, and Monkey's Paw until it responds. Don't just say a sentence and let go the talk button right away.

I'm not having issues with it in my experience. But I think the main things that troubleshoot the issue is:

1. Making sure you have Voice Recognition on at Options/Settings and clicking apply afterwards.

2. Make sure while you're using Voice Recognition, you're holding the talk button the whole time (if you use push to talk) when asking questions or saying sentences. Don't just say a sentence and let go afterwards.

there was this one time when i had a deogen and it spawned right on top of me when I was moving my crucifix. i thought "I'm gonna move it so it won't hunt on top of me" my flashlight was equipped so the crucifix went to my inventory. by the time i got the crucifix, deogen spawned right on top of me.
