The terminals for codes are fairly set, shadow zone is pretty much always in the same spot, and on charger side it's always in the same room, though it's a big room and there's a bit of a difference between it being right at the door vs deep in the room

And they are optional except for the main one

Nope, nothing. Every level is a self-contained set of objectives that never change minus maybe random pick up positions.
But the level tier acts as a difficulty settings, sort of.
A = Easy
B = Normal
C = Advanced
D = Hard
E = Very hard

Also from R4 and onward some missions will have optional objectives that might or might not make a level much harder. There are some C levels out there that hit you harder than some Ds

it's because of the construction of the game. All of the rooms, doodads, and enemies are stored in the game separately, and then the level using the blueprint that the designer created for it assembles it from scratch every time

This allows them to cut on map size, but unfortunately leads to the maps repeating rooms and often looking same-y
