if you crash, your dino is in the world for 5 minutes standing there idel for someone else to kill, starve or dehydrate. It's a different system to the "safe" logout you do when pressing "H"

Sometimes its a bug that it shows nothing, but will actually spawn you into a dinosaur when you load up the game. Usually something that happens when switching accounts

Otherwise, the game sometimes just randomly deletes dino saves when you log out, or maybe you got killed by someone whilst having crashed
