EVERYTHING you've got on your body has a bullet or two in it and the outline is really dark red. So your general condition (HP) is really low. He shot you in the leg or the hand and that was the tipping point to kill you
The only weird thing is that he wasn't clearly aiming at your legs or arm, but he definitely got a shot into either of those and the damage was enough to kill you. You were at death's door at that point.
