Nope, you don't need DLCs to play - but if you do, you're going to have more features - bigger map, customization options.

And well the biggest change is that there are real people trucking around you. There's one crazy road going between Calais and Duisburg, newbies would wanna stay away from there - but it's where majority of players roam all the time.
