This is something you will experience often in the group mode version of the fight. To prevent tiles being destroyed before the mechanic, you either have to burn down her HP to trigger the mechanic - before she summons meteors that would destroy a tile, or wait for the tiles to recover, then trigger the mechanic.

yes, youre most likely at the wrong spot. i havent done solo yet, but the way it works is:
if you deal enough stagger during the counter, he'll throw you 100yards away, and if not;
he'll bump you with his axe a few feet. for this, you need to stand below where his hand grabs the axe handle. if youre at the correct spot, you dont move at all
