some engravings (in this case, pm) dont really gain too much efficacy by increasing its level. However this is not the case for all engravings, keep in mind. For pm, the engraving itself does not really change the core playstyle nor offers any big incentives to have it higher, as in the scaling it has, is not that good. Thus its fine to leave it at lvl 1, and to have another engraving that gives u more damage compared to pm3

5x3+1 (pm) does more damage than 5x3 with lvl 3 pm and +1 ether pred or +1 adren

Your experience may vary depending on the class your building for and your stone.
But generally speaking yes, you need at least 2 6/3 accessories.
Your 5th engraving thats normal made from x3 +5s has to be made with x2 +6 and a +3
