it has:
a motorhome, home to a character
a place for the player to build their home
portal to an island with a victorian manor and a dark forest that's home to two characters
portal to a church island with a graveyard in it
portal to an entertainement or memory island ( i don't know which one to choose)

a tower

and a house that a portal to an island with all the EP's home (emotional parts)

and a small pond/waterfall used as a relaxation area

the common hall is located inside the tower

that's pretty much what i have in mind but there are also some stuff like a statue and stuff

Based on the video you already shared, I think you could make the transition from the central core to the various islands seamless, without a door frame, but with a sort of mental image shift.

Off the top of my head, I can think of Titanfall for its time-based transitions.

Titanfall 2 I believe, specifically.

The player had a phase device that allowed him to transition to different times in the same location.

Those transition sequences might help you out with your own.
There are other games that do similar things and I have another one on the tip of my tongue, but can't quite place my finger on it.
