You don't have to use it! But I recommend using it against certain demons like the archvile.

How hard is Nightmare difficulty?
It's honestly pretty hard in my own opinion, it requires that you actually understand the fundamental combat loop, but if you work at making sure you're not making common mistakes, you'll master it, as with any videogame, if you can't beat something, just keep at it, you'll get it eventually!
Common mistakes and things I'd recommend to people who are attempting this difficulty.

NEVER STAND STILL, this can work in extremely rare situation, but 99.5% of the time, it will get you killed. You want to make sure that you're constantly moving. Which brings us into point #2.

Make sure you are moving properly, movement in Eternal was something that I honestly didn't fully grasp the importance of until I started realizing that demons cannot hit you if you are moving properly. You can survive these arena's indefinitely if you are constantly moving and maneuvering in ways that make it so that you're constantly ducking behind walls or airborne, or are generally just too fast for them hit, (which by default, with dashing, you are already too fast for demons to hit in all honesty.)
So, master your movement, if you are struggling with any boss fight or just arena in general, learn how to move in a way so that the enemies can't hurt you.

Learn and experiment with different ways of dealing with demons. I would say a good example is realizing that you can defeat a dread knight by using a lock on burst, then instantly glory killing it afterwards. It's these small things that will make combat so much easier for you in the long run, because upon seeing certain demons, you'll have a quick and effective way of dealing with them.

Use. Every. Weapon. Tool. Item and Ability you have. I've had many instances where I forget that I have flame belch, or that my meathook can instantly give me armor if I use it to kill a fodder after hooking onto them. It's the small things like this that can really kill a run. So try to be wary of these things when you're fighting. I would say the #1 thing to keep in mind is that you should be constantly combining your tools, Like for example, you could just glory kill a fodder demon, but why not also flame belch it right before hand so that you get some armor? It's an example, but, it applies to many situations, similarly, why only flame belch that group of zombies? Why not ice bomb, then flame belch, then frag grenade for health AND armor. These things will really make a difference in the long run.

This is probably the most important point, take breaks, yes. Don't however just give up if things get challenging, there were many times during my nightmare run I felt like just giving up because certain advanced techs were too hard to do, or certain bosses felt impossible. Keep at it, you'll get it eventually as you're going.
I think that's it for my ted talk, honestly, but if you guys have questions about the game, I'd be happy to answer it! But keep in mind, I'm also still learning even after all the time I've spent playing it.

im number one on pc nightmare leaderboards horde mode
if you're on pc, i'd recommend checking out modded levels
there are some completely custom made, and some that are reallyy hard
randomizers also exist
and you can create custom difficulties with these commands
g_damageScaleAllToSlayer (multiplies damage to player by whatever amount you put here)
g_damageScaleAllToAI (same as the last but to demons)
ai_haste_overriderate (same, but for enemy speed)
