TDA shows it, with the advantage of modern hardware to not make it vague at all, and it's a major plot point in a much more story-driven game. It also has major consequences, with the next level and the intro to the one after dealing with what happens to him.
In Doom 1993 we actually physically walk into a Gateway (which the manual outright says these go to Deimos) and then the E1M8 title "Phobos Anomaly" is directly linked to the E2M8 title "Deimos Anomaly." Which also itself has a bunch of teleporters.

Realistically that one 3 second jumpscare is an abstract depiction of the entirety of E2M1. You just arrived on the other side of the Anomaly through the Gateway, and you fight a bunch of monsters.

Yes. To me that's just functional. The level ends without you pressing a switch. Technically speaking the level always makes sure to end before Doomguy can die.
I think just by the whole "they could've just given it a timer" thing, I also think, well, they could've just actually let the player die there, and then say "if the player dies while standing on this sector, end level a second later."

A pitbull doing that can easily lead to death, even if the dog didn't "mean to."
