The Cyberdemon is foreshadowed by the giant broken test tube amongst all the other test tubes of demons as well as Samuel and VEGA saying the argent accumulator was inside a “specimen”, the Hell Guards aren’t foreshadowed but you immediately know their purpose which is to guard the crucible which is all there is to them, the Mastermind is shown as a thing Olivia is merged with (and also cryptically shown via flashback cutscenes/hallucinations), the Crucible is established as being needed to close the The Well, and what the Wraiths are isn’t shown out of the codex which I agree with you there it should’ve been said but in terms of the lore of 2016 all you NEED to know is that they power the Well which is shown out of the codex
