you have to go to a dissonant planet, systems with such a planet will be tagged with that. go to the planet and find drills, you can see them in the analysis visor (icon is a thin upright stick looking thing), kill them until you get an item called echo locator, use it and you will find a camp. after that if you warp anywhere you will start the quest they who returned, introducing you to ||autophage||

I've found multiple sentinel ships and repaired them over the course of playing the game. And my experiences doing so have told me there's a common type for planets that have those ships on them. Stupidly high mountains covering every square inch, and it's on fire. The constant storms mean you can't even tell if there's a surface at all until you've run into it at full speed.
