Wormholes are currently not proven to exist. HOWEVER ; the string theory suggests that, if enough gravitational force is applied on two different spots in space-time, they will form a tear and can be connected. Alas, in order to keep this theoretical wormhole open, we'd need a special force to keep it open, which is known as exotic matter (comparable to anti-matter iirc)

On top of that, the paradox is that black holes will absorb any matter, so if there was one on each side of the wormhole, you'd essentially be stuck in a tear in space-time, you wouldn't really get anywhere due to the immense gravitational pulls (also probably forcing you to undergo spaghettification (yes, this is an actual term)). In order to counter that, we'd need a white hole (how charmingly creative) that spews matter out instead of consuming it, meaning it would need to extend FAAAR above space-time fabric to create a tear and thus connecting both black hole and white hole.
