I'm a QA tester and can confirm that most of the time bugs are NOT QA team's fault they're very often brought up to the dev team, and most of the time the fix priority is simply not high enough or dev resources are too low (without mentioning the cases where the bug is really hard to fix)
Even with a "very quick" fix, the build needs to be fully tested to make sure nothing else has broken, then sent for validation at console retailers....all that process easily takes 2 weeks IF YOU'RE LUCKY
