apply it to the boss on the opener, use it in line with your 2nd vendetta and then reapply it, use it in line with your 3rd vendetta and reapply it
then use it with your last tb before the boss dies as the final use (assuming no more vendettas)

Just do quick sim on raidbots and scroll down to the sample ability log to see what the sims is doing for your trinket setup.
As far as I know, you channel azshara prepull and before every vendetta, apply ashvane's first use when you can after that, and don't touch ashvane again until the final 20-30 seconds of the boss. That was one person's sim, your could be different

Then sim your character 2mn 1t Patchwerk with dung slice gear and with 5t patchwerk gear to better target what is a big loss and what is not.
You can sim more target (8+) if you know you will do crazy pulls and want to optimize theim but keep in mind this is relevant only for specific dung/pathing
Then decide wether you should opt for better ST/Prio target or full cleave depending on affixes

The macro is there to activate it at the same time as vendetta, to save you from having to press two buttons. Then as soon as the window's over, you use it (the trinket, not the macro) again to start gaining stacks again until the next vendetta

You don't only toggle between the two trinket modes by using the macro, the macro's just there to toggle the crit when you want it during the vendetta window. Outside of that, you manually toggle at the end of the window to recommence gaining stacks

To be totally optimal it's probably best to use it the GCD after Vendetta but it really doesn't matter too much
