1. Not using Deadly/Amplifying poison (or its not applied to your target)
2. Not using Rupture on your target

the 4pc tier set is a buggy mess after having been noted as “fixed” in a hotfix note. It has a chance to double proc, not proc at all, and sometimes even proc in the middle of the fight and desync all your cooldowns.
