you use clear the witnesses if you are not specced into vicious venoms. if you have vv, then you don't use FoK in single target (going from 6 to 7 combo points with FoK is roughly even to just slamming another VV mutilate in that scenario, too)

the two bosses hitboxes are close enough together it hits, but there are other checks for bosses with large hitboxes like gally
