there are weakauras that track exactly how long you still have time to use a sepsis ability before it runs out, that way you can use it at the last second of the buff both times

I use sepsis and rupture, ER, rupture, get in an envenom (or two if the first hits an echoing reprimand bonus) shiv, and then refresh the garrote before the next sepsis buff. I've basically changed my stealth bar when i'm raiding to be almost the same as my regular bar, so i don't accidentally hit a stealth ability and lose the sepsis buff. once you garrote and get it to 24 seconds, I use deathmark before the next sepsis buff.
So the second sepsis buff can use garrote when there's like 5 seconds left of the first refresh, getting you another 24 seconds of that good garrote. then you can vanish and garrote the next time you get to 5.
