Because of the hotfix addition of Sepsis to our Mastery list (read here: https://www.wowhead.com/news/puzzle-box-effect-nerfed-for-unholy-dk-patch-10-1-hotfixes-332843), Sepsis is now the best simming combination. Another surprise is that it doesn't seem too terrible to play around currently, as it doesn't require any special gameplay.

The following will be without exsanguinate, since exsanguinate requires dropping cut to the chase. This is a 0.4% gain (see the picture below), but I wouldn't recommend it to anyone who enjoys their sanity.

The way to play with sepsis in single target then is super easy. Your opener is as follows: Garrote -> Slice -> Sepsis -> Ambush -> Mutilate/Ambush to 5+ combo points -> Rupture -> Deathmark -> Shiv + Thistle Tea -> Echoing Reprimand -> rotation
PLEASE READ THIS I BEG YOU You’re still pressing vanish all the same after the first garrote gets into pandemic, just like normal. Simulations currently do not use vanish and will hold until 2min Deathmark. I have no reason to believe this will remain the case after optimizations. Within your actual rotation, you still sync vanish and deathmark, but you also will be pressing sepsis on cooldown, holding one of your shiv charges for it. In practice that will then look something like this (timings are rough estimates):

1:30 Sepsis -> Garrote, with Shiv
2:10 Deathmark + Vanish
3:00 Sepsis -> Garrote, with Shiv
4:10 Deathmark + Vanish
4:30 Sepsis -> Garrote, with Shiv
6:00 Repeat from 0:00```Sepsis here is in the Tier set bonus 1 out of 4 times, but that's fine, because we're using it on cooldown for the raw damage and stealth garrotes mostly. We want to prioritize our stealth garrotes as follows:
```True stealth > Vanish > Sepsis```If there is a "better" stealth garrote (when sepsis lines up during a deathmark), you want to use the stealth ability on sepsis for Ambush instead. Otherwise keep slamming Sepsis -> Garrotes on cooldown. The current best rule is improved garrote <14 seconds = apply a garrote. This means outside of vanish/Deathmark windows, you should be garroting the start and end of sepsis. **The difference between garrote and ambush from sepsis is really minor from first testing. I know you guys love overcomplicating the spec, but for real just push button. If you’re using the proc, you’re chilling**

If there is a "better" stealth garrote (when sepsis lines up during a deathmark), you want to use the stealth ability on sepsis for Ambush instead. Otherwise keep slamming Sepsis -> Garrotes on cooldown.
