Every item you get, can be upgraded (ilevel)

I'll give you S2 numbers since I don't know S3 yet:
If you upgraded an LFR item to 415, you'd gain the normal mode appearance of that item. If you upgraded a normal mode item to 427, you'd get the heroic mode appearance, if you upgraded an heroic item to 441, you'd get the mythic appearance of that item.

In theory, you don't need to raid mythic to get the mythic appearance

I'd love to tell you the item level threshold for the appearances but I don't know them yet

Now there are items from the rogue set that "do not exist". Those items will have a special way to get them, as you said: catalyst.

The best way to get all appearances is getting an LFR item and an heroic item of the slot you want.
