The best way to do this off the top of my head is to start farming for items on round 3. Get as many unique items as you can and use the duplicates you get to aid you in the round. they always have the same loadout, and are calculated to guarantee a win or loss after a certain amount of loadouts no matter the amount of cigarettes

second round is always 1-1, 2-2, 3-2, 3-3, 5-2
and by that point it's technically possible to go beyond, if you got perfect luck with cigs and beers
but I assume it would give a second 5-2s, and you mathematically can't go past that

Simulated Gambling would refer to something akin to a slot machine or casino but with an in game currency (see image 1).

To back this up, there is in fact a rating for AO titles for straight up gambling (see image 2).
