Sometimes your left hand doesn't hold your weapon when you halfsword and stuff, yeah. Your hand needs to be close to the blade itself to engage the initial grip, if that makes sense

So flailing your arms can cause you to drop it

More precise movements are what you wanna coin in

it's certainly not perfect by any means

and doing it in the middle of a fight means there is like a 20% chance of you not grabbing it

Another thing, with polearms and larger swords, hold right click and then aim the tip of the blade where you want, then double click left mouse for a much more stable stab. This is a realistic halfsword stabbing stance, and with some practice, can get you into the gaps in armour and stuff

I don't use alt too often, but I do agree with a lot of weapons it needs some work. Alt will alter the weapon functionality in many ways, mainly stabbing, and some overhead swings. I only really use alt to stab if it's with the small knives or Daggers. Try stabbing with that technique I just mentioned!
