if everyone is playing at their best and avoiding damage whenever possible, then the damage the group takes is very scripted and easy to figure out what to use when

Extremes you generally want to use Party Finder or ask friends/your free company/other community. You don't want to queue for extremes in Duty Finder like you would for dungeons

To actually answer the question, Trusts do not function in high end content at all.
Although there are plans to expand the Trust system through all the prior expansions MSQ dungeons and 4-man trials, Extremes do not fit into that category.

All Extremes on synch will still hurt if you're going in blind/with randos who don't know wtf they're doing. ARR trials hurt less than they used to because iLvl doesn't get synched the same way as character level, but even they will still pose a challenge. Im not there yet, but I assume stuff like EW Extremes will be rather difficult because modern content and no iLvl bloat. However, no time like the present to dip your feet in. Try an extreme or two (with fren or party finder) and give it a whirl

The difficulty of content depends on the skill you want to develop and the effort you're willing to put in. While extremes aren't difficult, they also require you to be able to read, interpret, and correctly position for mechanics that normal dungeons, raids, and trials will not throw at you.

Your macro would be something like this if you wanted it to properly queue up so that Life Surge is always used before Full Thrust
so unless you want to always be using the macro at a certain time in your GCD cycle, this would result in downtime for your damage, because you'd have about a second in which you aren't using anything

With this, due to the nature of how macros work without wait times, you'd have some instances where only Life Surge is used, some where only Full Thrust is used, and some where both are used.... which isn't consistent

If you're on PC you may just be clicking somewhere off the target to deselect them... I still do it sometimes
