Simply put up a random block in the biome where it can't exist normally (a spruce log in plains or packed ice in the desert)
If it's any of "simple" blocks - nothing would happen upon it's breaking. However, if it's a valuable one, like a block of gold - then upon breaking the block wouldn't drop, and a redstone torch would appear along with cave noise. Maybe also add a 50/50 chance of creeping Herobrine behind.

No idea if it's added as I only encountered HJALP redstone torches in my chests, but how about something new?
The item I want to suggest is called "Wake Up", and is either a piece of paper, or a whole book with a text of "Wake Up" copypasta in both English and Swedish languages from copeland streams. (you know, the one where they demand a player to WAKE UP and all? if you don't recall such text - I would send a link to the said text)
